Recovery & Post-discharge Home Care
Planning for a return home after being released from the hospital can be a happy yet stressful time. Lack of safety adaptation to the home, malnutrition and dehydration, errors in the administration of medication and inability to attend follow-up healthcare appointments can all result in hospital readmission. Don’t assume this responsibility alone. C.O.L.’s post-discharge care can assist in making the transition from hospital to home a pleasant one. Shorten your or a loved one’s stay in a rehabilitation center as our trained and certified Personal Care Attendants are eager to offer companionship, facilitate a full recovery and minimize any chance of readmission. Services include:
- Implementing a plan of care developed by your health care professional
- Scheduling and transportation to all follow-up medical appointments
- Management and assistance with self-administration of medication
- Assistance with personal care, household chores and meal preparation
- 24 hour warning sign monitoring and progress record keeping
Respite Care
We understand how invaluable your role is as a family caregiver. Your loved one has entrusted you to maintain his or her quality of life. As a result of your tireless dedication and commitment to your loved ones well-being at times you may experience physical, spiritual, or emotional fatigue. When this occurs C.O.L.’s trained and certified Personal Care Attendants are ready to provide you with the respite relief you deserve. Our first-class services are designed specifically to uphold the level of care and attention to detail that you have established as a family caregiver, while giving you the opportunity to go shopping, pursue activities of enjoyment, attend special events, or resolve an emergency situation. Our respite services include:
